VET AT THE BARN PATIENT FORMS

The staff at Vet at The Barn has assembled and posted some of our regularly-used forms for your use.

INSTRUCTIONS ON USE OF FORMS:
To view, print out, fill in and bring with you at the time of your next visit, simply click on the link next to the form you are interested in.
The forms are available in two formats: PDF (Adobe) or Microsoft WORD.
Selecting the format in the column listed WORD will result in the form opening up in your computer's Microsoft Word software (or a dialog box will
appear asking you to either OPEN or SAVE the form). Once you have the form opened, the
easiest way to use the form is to PRINT it and then fill it out by hand at your leisure.
Selecting a form in the column listed PDF will cause the form to appear in your computer's ADOBE READER viewer.

The optional method of using the online forms (if you have selected the WORD format) is to fill it in while it still appears on your computer monitor and THEN print it. However, in order to do this you must make sure your word processor is in the TYPEOVER mode rather than the INSERT mode. (There is a button marked "INSERT" on the right side of your keyboard). Once the word processor is in TYPEOVER mode rather than INSERT mode you can safely type the information on each line you put your cursor on, using your mouse (and NOT the TAB key) to locate the line on which you wish to type information.
